INSTRUCTIONS FOR BECOMING A CLIENT:
Connecticut Veterans Legal Center is a medical-legal partnership that works with veterans in treatment recovering from mental health issues, substance abuse, or homelessness. CVLC works with clients who are referred by their mental health clinician.
These three forms are necessary before proceeding to a screening interview where your legal issue and eligibility will be discussed.
- CLINICIAN INFORMATION FORM - To be completed by the clinician (See dropdown menu below for a list of acceptable clinician types)
- RELEASE FORM - To be completed by the veteran
- LEGAL PROBLEMS SCREENING FORM - To be completed by the veteran
You may either submit the forms below or you and your clinician can fax all 3 forms to us at 203-889-0111.
